Please use the appropriate IRS W-8 Form (W-8BEN Form / W-8BEN Instructions or W-8ECI Form / W-8ECI Instructions).

Once completed, please click for a Fax Cover Page and fax the cover page and completed tax form to 480-275-3996. Alternatively, you may email the completed form to taxforms@securepaynet.net. Please include the login name and payee name for your account with your fax or email.
