If you want to delete TVA, you must complete the following form of the VAT' exoneration and send it, attached to this mail.

On the document, at page "2", you have 4 boxes. On the first, you have to put your NIC, on the second you have to write your name and your first name, on the third, the name of your firm (if you have), and on the last, your signature and the firm stamp (if you have again).
With that document, you have to give a proof of address of your home or compagny.
